What is Peripheral Artery Disease (PAD)?

Peripheral Artery Disease (PAD) or Peripheral Vascular Disease (PVD) is a common disease related to the blood circulatory system in the body. In this disease, the arteries in the body connected to the head, stomach, legs and arms are affected. In this case, the word ‘Peripheral’ means away from the heart or the outer regions of the body. Peripheral artery disease (PAD) is similar to coronary artery disease (CAD) in the sense that both of these disorders are caused by atherosclerosis (plaque build-up in the arteries). In atherosclerosis, arteries in the body get narrowed to the point that it causes blockage in the blood circulation and complications especially in the critical regions in the body.

 

How does PAD affect a person’s body?

When a person develops PAD disease, the extremities of the body are the usually most affected areas. One of the most common PAD symptoms is the build-up of plaque in the blood carrying arteries. The plaque is usually made up of cholesterol, calcium, fat, fibrous tissue and other substance in the blood. As this disease creates blocks in the blood circulatory system, most of the extremities do not receive sufficient blood required for their maintenance. This is most notably observable in the legs as the patient starts to develop leg pain.

Symptoms of PAD

A lot of the patients suffering from PAD may have no symptoms at all. However, in some cases, the patients might develop excruciating leg pain when they walk, which might disappear as quickly as it came. This phenomenon is medically termed as ‘intermittent claudication’. This pain may range from mild to severe and may go away after a while when the leg is given some rest. Sometimes both the legs are affected at the same time though the pain in each leg may not be the same.

Peripheral artery disease (PAD) symptoms develop slowly over time. If the symptoms of the patient are sudden in nature or get unexpectedly worse in a short duration, then the symptoms might be indicating to a much graver problem requiring immediate treatment and resolution. With the progression of the disease the leg pain may occur even when the leg is at rest. This is known as ischemic rest pain. Some of the symptoms that are observed in this disease include:

  • Weakness or numbness in the legs
  • Slow growing and brittle toe nails
  • Hair loss on the legs and the feet
  • Change In skin colour on the legs such as turning pale or sometimes even blue
  • Shrinkage or wasting away of the muscles in the legs
  • Ulcers and/or open sores on the legs and/or the feet that don’t heal
  • Erectile dysfunction (Men)
  • Skin that is shiny
  • Feeling of cold in the lower foot or leg, especially when compared to the other side
  • Weak or no pulse in the legs or feet/foot
  • Painful cramping of the thighs, hips, calves that come after straining activities like walking and/or climbing stairs

On the basic level, this disease can be tackled with regular physical exercise, staying away from smoking cigarettes and adopting a heart-healthy food lifestyle for a life-long duration. This procedure is performed to bypass of circumvent the affected area of the arterial vasculature. If the vessel is not available or is not adequate enough, then an artificial synthetic tube is often used for long grafts for the grafting procedure instead

Angioplasty

This procedure is also sometimes referred to as percutaneous transluminal angioplasty and can be performed on solitary lesions in large arteries (e.g. femoral artery), but may not have a pertaining benefit. A catheter with a balloon at the tip is inserted into the blocked artery. The balloon is then inflated causing the plaque to be pushed outwards of the artery wall widening the artery and restoring the blood flow. A stent may also be placed in the artery during angioplasty. A stent helps in keeping the artery open even after the surgery. Sometimes the stents are covered with medication to prevent further blockage in the artery. The rate for the artery to be open for long is better in the cases for the arteries of the heart (or iliac arteries) and decrease with the arteries towards the legs

Atherectomy

In this surgery, a catheter (small tube) is inserted into the blocked artery to remove the plaque build-up. This catheter has a small cutting device inserted in it that cuts off or shaves off the plaque that is blocking the artery. This helps in restoring the normal flow of the blood. The pieces of plaque in the artery are then removed through the catheter or get washed away in the blood stream (if they are small pieces). Atherectomy is sometimes performed using laser that can dissolve the blockage in the artery

Thrombolysis and Thrombectomy

If there is a development of arterial thrombosis or embolism, then thrombolysis and thrombectomy are used to tackle with this problem

Amputation and Sepsis prevention

Due to the lack of blood flow, some parts of the body may develop gangrene due to infection or just a general lack of blood. In such cases where, a part of the body is gangrene ridden, the affected area is usually amputated to avoid the spread of infection and thus preventing sepsis as well
